It is an interesting observation that during my day to day consultation with my patients I have found every third person to be suffering from Diabetes and almost every alternate patient has got somebody in their family who is suffering from Diabetes. This extremely high prevalence of Diabetes is evident only in our country.
The management of Diabetes is getting eased off over last few decades due to immense research work which is being done in the discipline of Diabetes throughout the globe. In spite of so much of development, hurdles remain profound in managing Diabetes. These hurdles are ignorance and ill –information regarding Diabetes among citizens and foremost of all are the misconceptions regarding Diabetes.
I can presume that whichever Physician a patient consults, he gets quite a bit of information regarding management of Diabetes and in most of the cases it is quite unlikely that there will be any difference of opinion. The problem remains in dedication of adequate time to explain each and everything about Diabetes which is hardly given by any physician nowadays. Even I take 45 mins to examine and give adequate consultation to a Diabetic patient. All the physicians want their patient to keep on doing well without any ups and downs. But even then, be due to inadequate sharing of information from the physicians part or due to negative typecasting of the doctor fraternity by the media, it is hard to find a patient to sustain his/her treatment with a single Physician. The trust never develops and that is quite unfortunate. If one does not have adequate confidence on the qualification and credibility of the doctors’ management skills then the overall treatment gets a hard beating.
The motto behind authoring this Diabetes Manual is quite simple. It is to make things very clear and lucid regarding Diabetes. 
The size of the manual is quite large and it is being delivered with basic information regarding Diabetes. The adverse events of Diabetic Medicines have been incorporated and also I have ensured that the Targets in management of Diabetes is also well elucidated which is a very important information. Facts and facets regarding Insulin should be very clear. Most of the people are afraid of Insulin. The two most important reasons behind this fear is first of all it is in injectable form and most of us are afraid of injections and secondly a myth is imbibed among the common people that usage of insulin is a tell-tale sign of end stage of Diabetes which is actually a big negative idea. Even it took pains to break the ice while initiating insulin to my mother who is a 12 year long diabetic and that now when her blood glucose is under control, she is disbelieving the laboratory reports.

  • Almost 1 out of 2 patients will be found suffering from Diabetes related complications at the time of Diagnosis of diabetes for the first time.

WHAT IS DIABETES?

Diabetes is a disease in which blood glucose levels are above normal. Most of the food we eat is broken down into glucose, or sugar, which are oxidised by our tissues to produce energy. The pancreas, an organ that lies near the stomach, secretes a hormone called insulin. Insulin pumps in glucose from our blood into the cells of our bodies. When you have diabetes, your body either doesn't make enough insulin or can't use its own insulin well as it should. This causes sugar to build up in your blood. 
Diabetes can cause serious health complications including heart disease, blindness, kidney failure, and lower-extremity amputations. There is no diabetes cure developed so far.

HOW BLOOD SUGAR IS NORMALLY CONTROLLED IN OUR BODY?

The Blood Sugar is tightly regulated between 70 to 110mg/dL in fasting state and 100 to 140mg/dL in post meal state. At any random state Blood Sugar is maintained below 180 mg/dL. This is done by the balanced action of some hormones; some of which tend to increase blood sugar while some tend to decrease.Hormones that increase blood sugar are Glucagon, steroids, Adrenaline, Somatostatin, Thyroid hormones, Growth Hormone etc. 
The only hormone that reduces blood sugar is the INSULIN. Whenever there is a tendency of the blood sugar to rise above the permissible level, Insulin is secreted from the pancreas and the sugar from the blood is pushed inside the cells by insulin and the balance is achieved.

WHAT IS INSULIN?

Insulin is a protein hormone secreted from Pancreas. It is released from pancreas in two patterns.A constant secretion of insulin takes place throughout the day irrespective of meals which ensures that blood sugar in fasting state is under limits. This continuous Insulin secretion pattern is called the BASAL insulin.
On the top of this whenever there is any intake of food, Insulin senses the transient rise in blood sugar and is secreted in a pulsatile manner with each meals to bring down the post meal rise in blood sugar. This pulsatile pattern of Insulin release is called PRANDIAL Insulin.
When the doctors treat patients with insulin, they always try to match the above mentioned pattern of Insulin levels in the blood which is often difficult.

WHAT ARE MY RISKS FOR DEVELOPING DIABETES?

Roughly it can be extrapolated from researches on Diabetic genes that,
  • If your mother is Diabetic: You have 19% chances to develop diabetes
  • If your father is Diabetic: you have 14% chances to develop diabetes
  • If both the parents are Diabetic: You have 25% chances to develop diabetes
  • If your sister or brother is Diabetic: You have 75% chances of developing diabetes
  • If your twin is Diabetic: You have 99% chance of developing diabetes.
WHAT ARE THE SYMPTOMS AND SIGNS OF DIABETES?
People who think they might have diabetes must visit a physician for diagnosis. They might have SOME or NONE of the following diabetes symptoms
  • Frequent urination
  • Excessive thirst
  • Unexplained weight loss
  • Extreme hunger
  • Sudden vision changes
  • Tingling or numbness in hands or feet
  • Feeling very tired much of the time
  • Very dry skin
  • Sores that are slow to heal
  • More infections than usual.
  • Nausea, vomiting, or stomach pains may accompany some of these symptoms in the abrupt onset of insulin-dependent diabetes, now called type 1 diabetes.

WHAT ARE THE TYPES OF DIABETES?

Type 1 diabetes, which was previously called insulin-dependent diabetes mellitus (IDDM) or juvenile-onset diabetes, may account for about 5% of all diagnosed cases of diabetes. 
Type 2 diabetes, which was previously called non-insulin-dependent diabetes mellitus (NIDDM) or adult-onset diabetes, may account for about 90% to 95% of all diagnosed cases of diabetes. 
Gestational diabetes is a type of diabetes that only pregnant women get. If not treated, it can cause problems for mothers and babies. Gestational diabetes develops in 2% to 10% of all pregnancies but usually disappears when a pregnancy is over. 
Other specific types of diabetes (like diabetes insipidus) resulting from specific genetic syndromes, surgery, drugs, malnutrition, infections, and other illnesses may account for 1% to 5% of all diagnosed cases of diabetes.

WHAT ARE THE RISK FACTORS FOR DIABETES?

Risk factors for type 2 diabetes include
  • Older age,
  • Obesity,
  • Family history of diabetes,
  • Prior history of gestational diabetes,
  • Impaired glucose tolerance,
  • Physical inactivity, and
  • Race/ethnicity. African Americans, Hispanic/Latino Americans, American Indians, and some Asian Americans and Pacific Islanders are at particularly high risk for type 2 diabetes.
  • Any history of Polycystic Ovarian Disease in females (PCOD)
  • History of stress 
Risk factors are less well defined for type 1 diabetes than for type 2 diabetes, but autoimmune, genetic, and environmental factors are involved in development of this type of diabetes.
Gestational diabetes occurs more frequently in African Americans, Hispanic/Latino Americans, American Indians, and people with a family history of diabetes than in other groups. 
Obesity is one of the associated causes of diabetes with higher risk . Women who have had gestational diabetes have a 35% to 60% chance of developing diabetes in the next 10–20 years. Other specific types of diabetes, which may account for 1% to 5% of all diagnosed cases, result from specific genetic syndromes, surgery, drugs, malnutrition, infections, and other illnesses.

WHAT ARE THE COMPLICATIONS OF DIABETES?

Most of the complications of Diabetes are chronic. High blood Glucose tends to damage numerous enzymes and proteins of our body necessary for important functions and hence many organs are ultimately damaged. We called this as TARGET ORGAN DAMAGE (TOD).
The important target organs are Kidneys (Both), Eyes, Heart, Brain, Nerves and Arteries.

According to Ayurvedic texts, mostly, too much of heat generated in the body (or pitta) leads to headaches. In such situations, cooling the body down may help reduce the pain. Drink some buttermilk or tender coconut water; ot try massaging some castor oil onto your scalp and the soles of your feet. You can then follw this up by washing them with warm water and shikakai. Apart from this you could also try including poppy seeds (khus-khus), coconut and ghee into your diet.

Foods such as cheese, coffee, chocolate and tea that contain vasoactive amines, that are known to trigger painful migraine headaches. These amines affect the flow of blood in and around the brain, leading to headaches. If you frequently suffer from headaches, maintain a food diary and note down what food specifically triggers your headaches and avoid these foods to keep headaches at bay.

Manage stress levels-

One way to help reduce stress is by planning ahead and organizing your day. Another way is to allow more time to relax. And if you're caught in a stressful situation, consider stepping back.
Go hot or cold. Applying heat or ice — whichever you prefer — to sore muscles, may ease a tension headache. For heat, use a heating pad set on low, a hot-water bottle, a warm compress or a hot towel. A hot bath or shower also may help. For cold, wrap ice, an ice pack or frozen vegetables in a cloth to protect your skin.

Perfect your posture-

Good posture can help keep your muscles from tensing. When standing, hold your shoulders back and your head level. Pull in your abdomen and buttocks. When sitting, make sure your thighs are parallel to the ground and your head isn't slumped forward.
Following home remedies have proven helpful to get rid of headache's-

1. Ginger

Ginger can help reduce inflammation of the blood vessels in the head, thus providing relief from a headache.Mix equal parts of ginger juice and lemon juice. Consume it once or twice a day.
Another option is to apply a paste of one teaspoon of dry ginger powder, also called sonth, and two tablespoons of water on the forehead for a few minutes.
You can also boil ginger powder or raw ginger in water, and inhale the vapor.
Chew one or two pieces of crystallized ginger candy.

2. Mint Juice

Menthol and menthone are the primary components of mint which are very effective in alleviating headaches.
Extract mint juice from a handful of mint leaves and apply it on forehead and temples to treat headache.
You can also place mint tea compresses on the forehead to relieve discomfort. Along with mint juice, corainder juice is also effective in treating headaches.

3. Peppermint Oil

Peppermint contains menthol that helps open up clogged blood vessels that cause headaches. It also has calming and soothing effects.
Mix three drops of peppermint essential oil in one tablespoon of almond oil, olive oil or just water. Massage your forehead and temples with it. Or, you can put crushed fresh peppermint leaves on your forehead.
Alternatively, you can prepare a steam treatment by adding a few drops of peppermint essential oil to a small pot of boiling water. Inhale the steam for a few minutes.

4. Basil

Basil works as a muscle relaxant, making it a helpful treatment for mild headaches caused by tense muscles. Plus, it has calming and analgesic effects.

Put three or four fresh basil leaves in a cup of boiling water and let it simmer for a few minutes. Optionally, add a little honey and then sip the tea slowly.
You can also boil one tablespoon of basil leaves or a few drops of basil oil to a pot of boiling water and carefully lean over the pot to take in the steam.
Alternatively, chew some fresh basil leaves or massage your forehead with basil oil mixed with a base oil.

5. Lavender Oil
Simply smelling the soothing scent of lavender essential oil can be of great help in relieving tension headaches. Research suggests that it can also help improve migraine symptoms.

Put a few drops of lavender essential oil on a tissue and inhale it. You can also add two drops of lavender oil to two cups of boiling water and inhale the steam.
Another option is to mix two or three drops of lavender essential oil in one tablespoon of carrier oil such as almond oil or olive oil and massage your forehead with it.
Note: Do not take lavender oil orally.

Too embarrassed to deal directly with doctors on sexual and psychological problems, a lot of Indians are logging on to virtual clinics for faceless medical advice



Vinay Singh (name changed) could never muster the courage to talk to "doctor uncle" about his bedroom woes. The 28-year-old Noida-based entrepreneur suffered from premature ejaculation for months till he stumbled upon a mobile app which offers 'private and anonymous consultations' with 'qualified' doctors.

Singh immediately downloaded the Mediczy app, registered with just his initials as user id and shared his concern. Within 2 Minutes, ten sexologists responded with advice. He started a telephonic consultation with the one whose suggestions he found most practical without showing his identity. "Thanks to him, I managed to overcome the problem," he says.
Singh is among a growing number of Indians who are turning to mobile-and-web consultations to seek help for conditions they are too embarrassed to discuss face to face with doctors.

Mediczy App allows you to consult India's leading Doctors and Specialists from anywhere.

 Mediczy, a healthcare startup which offers app-based voice and video medical consultations, is aiming for in its very first year of operations. And that too with only 600 doctors on its panel.
While that may seem a little ambitious at first glance, Mediczy has a ‘magic pill’ for that.
“We intend to generate demand and match that demand with the number of doctors needed. We are not a marketplace and so the number of doctors we want to empanel is not that important for us,” says Suresh Chandrasekar, founder and CEO, Mediczy.
Doozyfive Technologies Pvt Ltd, which runs the app, has 4,000 registered users and has received mass unique visitors on the site till now. The healthcare startup currently charges a flat fee of as per doctor requirements from patients. The startup, which had a soft launch late in 2015 recently looking  funding of $500,000 (Rs 3.31 crore) from angel investor/ Ventures capital firm.

The Prescription
While random patients will form part of its customer base, Mediczy is banking on its B2B2C model to scale up its business via tie-ups with employers.
“We have incorporated the PMPM (per member per month) approach wherein a company will pay a very small fee and in return the employees will get a deep discount on consultations.
Mediczy is also in talks with some bigger companies and aims to have 100-200 clients on board in the next one year.
A face-to-face consultation with a doctor at a private hospital in a metro can cost anything up to Rs 1000 in comparison.
Reading the pulse
Mediczy has a panel of physicians, pediatricians, psychologists and nutritionists available round the clock. Anyone having a sudden health issue or searching for a second opinion can access doctors via the Mediczy app. Patients can connect to a doctor instantly or schedule an appointment for a call/voice chat/video chat. The fees can be paid via online payment channels Payumoney.

“Rising mobile and internet penetration in India has made the market mature for telemedicine in the country,” says Suresh.
